- Obverse
- In the upper part of the field, image of the Eagle established as the State Emblem of the Republic of Poland. Above the Eagle, semicircular inscription: RZECZPOSPOLITA POLSKA. At the sides of the Eagle, notation of the year of issue: 20-11. Under the Eagle, on the right, inscription: 200/ZŁ. Below, stylised images of: a sabre with the inscription “Honor i Ojczyzna” and a fragment of a lance. Under the Eagle’s left leg, the Mint’s mark, M/W. (Republic of Poland Honour and the Motherland)
- Reverse
- In the centre, against the background of stylised silhouettes of mounted uhlans, a stylised image of a mounted uhlan holding a sabre. Above, semicircular inscription: UŁAN II RZECZYPOSPOLITEJ (Uhlan of the 2nd Polish Republic)
