- Obverse
- The Polish eagle with the date below, the value at the bottom of the outer ring, and the country name divided from the value by two dots. The mintmark above 9 in date. (REPUBLIC OF POLAND MW 2019 5 ZLOTYCH)
- Reverse
- The Liberation Mound, engraver's initials at the lower right, latent image at the bottom (LIBERATION MOUND GP NBP / 2019)
- Edge
- Irregularly reeded with inscription: “NBP”, repeated eight times, every second one inverted by 180°, separated by stars.
